Incident report

What is the purpose of this form?

This is a place for community members to report consent and safety incidents directly to HD Latin Dance leadership.

• You can report incidents of dangerous dancing, sexual harassment, discrimination, or anything else that violates our Code of Conduct or makes you feel unsafe.

• You can report something you witnessed happening to someone else.

• You can have a friend submit a report for you and act as a third party go-between.

What happens after I submit a report?

After you submit a report, we will reach out to the reporter (if they consent) to have a conversation about the incident. From there, we will determine what action is most appropriate based on the nature of the incident and the reporter’s wishes.

Anonymity

• Reports are confidential and anonymous (we will not disclose the name of the person who made the report, or the people involved in the report).
